THE LIST OF DOCUMENTS
SUBMITTED TO THE ADMISSION COMMITTEE
FOR ENTERING THE ACADEMY

• identity document (original and Xerox copy);
• state-issued document on education (original and photocopy); the original must be submitted when making a decision on enrollment;
• photos 3x4 cm format (color or black-and-white, on thick photo paper, photos must correspond to the age of the applicant):
for registration of the applicant’s personal file – 2 pieces,
for admission to preparatory courses –1 piece,
for registration of student documents –3 pieces,
for registration of a student ID into the dormitory – 1 piece;
• application (to be filled in at the Admission office of the Higher school of folk arts);
• document confirming the existence of individual achievements;
• a copy of the marriage certificate and (or) a certificate from the civil registration authority (if the surname, first name, patronymic does not match in the documents);
• military ID card – for men (original and Xerox copy);
• documents confirming the existence of special rights (original and Xerox copy);
• documents confirming the right to enter out of competition upon condition of successful passing entrance exams or a pre-emptive right to admission (original and Xerox copy);

Medical certificate (form 086-U), certificate of vaccinations are provided at the Higher school of folk arts when enrolling.

All the documents must be translated into Russian and certified by the competent authorities.
